The pain of patients: a priority to care for

ABSTRACT

Objective: To describe the level of knowledge and attitudes of patients about pain management in adult patients in different hospital services in high-level institution in Bogotá, Colombia.
Materials and methods: A descriptive cross-sectional study was carried out in which 85 nurses from a high complexity hospital in Colombia. Nurses participated voluntarily in emergency, hospitalization, surgery and critical care services. Demographic characteristics, professional experience and academic training were analyzed. The level of knowledge and attitudes about pain management was evaluated through Knowledge and Attitudes Survey Regarding Pain (KASRP) questionnaire in Spanish version. This questionnaire includes 37 questions that are grouped into 4 domains: cancer pain, pain assessment, pharmacology, and substance abuse; and physical dependence.
Results: 66 % of the participants were women and the average age was 34.2 ± 7.3 years. 17.6 % have a specialization and 34.1 % have a master’s degree. 52.9 % reported formal training in advanced pain management. On average, the nurses achieved a score of 53 correct responses to the scale. This result is in acceptable range of (50-75 %), showing greater proximity to the limit of poor knowledge.
Conclusions: Most of nurses are young professionals with high academic level and clinical experience. However, even though most reported having advanced training in pain management, the results of this study show significant gaps in knowledge and attitudes about pain management. These knowledge and attitudes can be improved through training actions that include problem-based learning method in undergraduate and postgraduate programs and continuous and multidisciplinary training.



RESUMEN

Objetivo: Describir el nivel de conocimientos y actitudes de un grupo de profesionales en enfermería sobre el manejo del dolor en pacientes adultos en los diferentes procesos hospitalarios de una institución de cuarto nivel en Bogotá, Colombia.
Materiales y métodos: Estudio descriptivo de corte transversal en el que participaron de manera voluntaria 85 profesionales en enfermería de un hospital de alta complejidad en Colombia en los servicios de urgencias, hospitalización, cirugía y cuidado crítico. Se analizaron las características demográficas, experiencia profesional y formación académica, y se evaluó el nivel de conocimientos y actitudes sobre el manejo del dolor con el cuestionario Knowledge and Attitudes Survey Regarding Pain (KASRP) en su versión en español. Este incluye 37 preguntas que se agrupan en 4 dominios: dolor oncológico, evaluación del dolor, farmacología y abuso de sustancias, y dependencia física.
Resultados: El 66 % de los participantes eran mujeres y la edad promedio fue de 34,2 ± 7,3 años. El 17,6 % tenía especialización y el 34,1 % maestría. El 52,9 % refería haberse capacitado formalmente en manejo avanzado del dolor. En promedio, los enfermeros lograron una puntuación de 53 respuestas correctas ante la escala, lo cual se encuentra dentro del rango aceptable (50-75 %) y muestra mayor cercanía al límite de bajo conocimiento.
Conclusiones: La mayoría de los enfermeros que participaron en el estudio son profesionales jóvenes con alto nivel académico y experiencia clínica. Sin embargo, a pesar de que la mayoría refería contar con estudios avanzados en el manejo de dolor, los resultados de este estudio evidencian vacíos significativos de conocimiento y actitudes sobre el manejo del dolor, que pueden ser susceptibles de intervención a través de acciones formativas que incluyan una metodología de aprendizaje basado en el problema en los programas de pregrado y posgrado de enfermería, así como espacios de formación continua y multidisciplinaria.
