Síndrome de Parsonage-Turner: presentación atípica posterior a vacuna para COVID-19. Reporte de un caso y revisión de la literatura

RESUMEN

El síndrome de Parsonage-Turner (SPT) se define como una neuropatía periférica axonal de comienzo agudo. Con mayor frecuencia se caracteriza por dolor severo, paresia progresiva unilateral en los miembros superiores y atrofia muscular. Es incapacitante en su fase aguda y puede producir discapacidad prolongada, que podría reducirse con un tratamiento oportuno. El SPT adquirió relevancia en el contexto de la pandemia por SARS-CoV-2 ya que ha sido asociado con múltiples desencadenantes que incluyen infección viral y vacunas, pero debido a su baja frecuencia suele no diagnosticarse. Aunque la electromiografía y la neurografía por resonancia magnética son útiles para confirmarlo, la clínica continúa siendo el método diagnóstico más fácil, confiable y costo-efectivo. Presentamos un paciente con SPT posterior a la vacuna contra COVID-19, cuyo diagnóstico inicial se basó en los hallazgos clínicos y la electromiografía. Se realizó además una revisión de la literatura relacionada.



ABSTRACT

Parsonage-Turner syndrome (PTS) is described as an acute onset axonal peripheral neuropathy. Severe pain, progressive unilateral upper limb paresis and muscle atrophy are common features. It is disabling in the acute phase and can lead to long-term disability, which can be reduced with prompt treatment. PTS became relevant in the context of the SARS-CoV-2 pandemic as it has been associated with multiple triggers, including viral infections and vaccines, but due to its low frequency it often goes undiagnosed. Clinical examination remains the simplest, most reliable and cost-effective diagnostic method, although electromyography and magnetic resonance neurography are useful for confirmation. We present a case of PTS following COVID-19 vaccination. The initial diagnosis was based on clinical findings and electromyography. A review of the relevant literature was also undertaken.
