Abstract

Complex regional pain syndrome (CRPS) is characterized by chronic regional pain disproportionate to the triggering event, associated with sensory, autonomic, motor, and trophic alterations. We present the case of a 57-year-old man with CRPS type I affecting the right upper limb following a traffic accident, with a 19-year course characterized by refractory pain and severe motor manifestations, including dystonia, tremor, and marked functional limitation, leading to impaired ambulation due to instability, motor clumsiness and marked emotional deterioration. The patient received pharmacological treatment and neuromodulation through spinal cord stimulation, achieving partial pain relief without significant improvement in motor symptoms. This case allows a review of the underlying pathophysiological mechanisms, available therapeutic options, and the importance of differential diagnosis, highlighting the need for an early and intensive multidisciplinary approach to reduce disability and improve quality of life.
